One of New York’s strangest secrets sits on Roosevelt Island: the crumbling remains of the Smallpox Hospital, built in 1856 to isolate and treat victims of one of the deadliest diseases of the 19th century.

Designed by architect James Renwick Jr. (who also designed St. Patrick’s Cathedral), the Gothic Revival building was considered advanced for its time — focusing on ventilation and patient separation to slow infection spread.

The hospital later became a training facility for nurses and eventually fell into ruin after smallpox vaccinations made the disease far less common. Today, the eerie stone shell still stands — fenced off and preserved as a landmark — quietly reminding New Yorkers of a time when epidemics reshaped the city.
